Take a look around before you pick a Tattoo parlour to go to - my first parlour was suggested by only one friend and was supposed to be "awesome" instead it turned out to be a dive, and it hurt like hell - the guy doing it pressed the needle in WAY to hard and drew blood throughout the entire thing. My second and third Tattoos however I had done at another parlour closer to where I am now - and I had asked about 20-something people about how good they where, and got nothing but good reviews.
Melanie, my tattoo artist now has one hell of an eye for detail, has never caused me any pain beyond an odd tickling sensation and I would recommend to anyone.
As for your first tattoo - pick a design you aren't going to look back on in five years time and go "Why the hell did I have that put on me?" something with a strong, awesome and personal meaning - on my right shoulder I have one of the army insignias from a webcomic I ran years ago, on my left arm I have the coheed and cambria dragonfly on a starstriped background as a sleeve around my entire lower arm/wrist - and on my wedding finger I have my wife's name in standard galactic alphabet.
Preparation? Eat a HUGE bowl of pasta about an hour before you have to go in - this is a serious must, your bodys pain reflex will kick in during the tattooing and it will digest the living fuck out of any calories and sugars while you are in the chair - I also recommend whatever high sugar sweets and energy drinks (note sugar - not caffeine) and sit there nomming your way through the entire experience, on your way out I can tell you it will hurt depending on your pain tolerance levels - but if you eat the right stuff before and during, you will walk out feeling like a BAUSE afterwards.
Afterwards - Tattooing is addictive as hell, give it about 6-9 months later and you will be thinking of your next idea - and then you'd probably have your second tattoo the year after the first and so on.
I myself am already thinking about what the hell I am going to have as tattoo number four - I am thinking maybe a small red lego 2x2 brick on one of my shoulders on the back.
